Schedule an appointment:
Other than calling us at 940-566-5714, you can schedule a consultation by contacting us online using our Request an Appointment form. We will contact you within 24 to 48 business hours from the time of your request to confirm the date and time of your appointment. If at any time you are unable to keep your appointment, please notify us at least 24 hours in advance so we can reschedule your appointment at a more convenient time.
Patient Forms:
Once your initial appointment has been scheduled, please download and print the following forms and complete them prior to your arrival. This will save time at registration and get you in the therapist’s office more quickly.

Billing:
We accept cash, checks, Visa and MasterCard and will file all of your claims for you.
We can also set up a payment plan for any remaining balance.
WE ALWAYS, ALWAYS, refund any money to you once your account is settled with your insurance.
We will be glad to discuss your deductible, co-pay and insurance plans with you and will verify your benefits before making your appointment.
